(2) Elissa Cano start date was Oct. 1, 2021 and her background check was withdrawn on Nov. 3, 2021. She did reapply and it was cleared on Nov. 16, 2021. The Director should have been monitoring the background check to follow up to make sure it was completed in appropriate amount of time, which is 7 to 14 days. It took over 40 days for this person to submit a correct background check. 3) Staff Forte, Ver Balin and Cano could not provider proof of TB at the Nov. 22, 2021 virtual monitor inspection.
